Clicking on the Page Header button or clicking and dragging it into your survey page will create a heading for your survey page, allowing you to split the survey into sections. Think of this as a chapter marker. This element can also be used as a footer.
You can add some text beneath the page header by checking the box next to Include a note. When you click on that checkbox, a textbox appears that you can input your text into. You might want to include a note to tell participants a little more about your survey, display your company’s tagline or even add the date.
You can click on the Media button to upload an image or embed video into the page header. This is a great way to include your logo in your survey. Be sure to click on the blue Done button when you’ve finished editing, so that your changes take effect.
